As per customer service, it is only for the gift cards shown: Macy's, Lowes, Spa Finder and Mastercard. Mastercard GC's have an activation fee of $5.95 so your total MM will be $14.05.

plus if you have 5 or 6 % grocery card the activation fee is basically a wash

Bear in mind that it can take 24 hours until you see on any reciept that you qualified.

Is this limited to 1 per person? Otherwise unilimited profit, no probelm using Square as the MC gcs are anonymous

Neither says that you need to use a MC

There is a second promo on the online link that has to do with using a MC
