Aeroplan - Flat Fee per booking $50 (coach), $100 (Business), $125 (First) All the these prices are CAD. Plus tax.  - or can use miles (bus was 10k so assume @ 100/$1 for any)
Alaska Mileage Plan- Does not issue infant tickets.
American - 10% of full fare
British Airways - Taxes and 10% of miles
Lufthansa/Miles & More - Taxes (Usually $17.50)
United - 10% of full fare
US Airways - 10% of full fare
singapore- 10% of full fare (when flying Sq you could find a date showing a discounted fare)
ANA?
IB - 10% Full fare
Asiana- 10% of the adult mileage ticket, plus taxes & fees

Why wrong DOB? I'm allowed to book a seat for infant no?!

Wonder what chances are I can ask for bassinet then... would probably make things harder...  but 32500 UA miles may still be better than $550...

Also UA said some very very weird thing, that they can only ticket the lap child 48 hours before departure, because it's multiple carriers, I said never had a problem but they said that department requires that travel date be less than 48 hours..
You can book a seat for infant.

I would HUCA UA and get the lap child added.

I had similar last week add regular UA reps simply couldn't get my daughter ticketed because it's partner airline bla bla bla and needs special department bla bla bla, after a few days I just used Dan's callback link and they ticketed right away!

I believe you still need to pay taxes on infant.

I tried that and swiss wouldn't do it for me. They said only united could ticket the infant

I just thought it would be a little weird to book the baby a seat on her own. Anyways, after calling up a few more times and getting different quotes in the upper $2000's, I got an agent who charged me $400 for the baby. Love how each US agent had something different  :P

They'll give it to you for free. It says on the web site, and I'm speaking from experience (united i imagine), but you have to call up before to secure it, or they probably won't have it available when you check in. They give you 2 seats in the front row for traveling with an infant.

traveling from NYC to MEL with Qantas booked it with aa miles ,
where is  better to add the infant Qantas or american ?
any idea how much they'll charge me?

I remember reading they're more baby friendly on award tickets but make sure to confirm.
On a side note, don't take it for granted that you can add your infant through the operating carrier. I never had success doing that.
