Aeroplan - Flat Fee per booking $50 (coach), $100 (Business), $125 (First) All the these prices are CAD. Plus tax.  - or can use miles (bus was 10k so assume @ 100/$1 for any)
Alaska Mileage Plan- Does not issue infant tickets.
American - 10% of full fare
British Airways - Taxes and 10% of miles
Lufthansa/Miles & More - Taxes (Usually $17.50)
United - 10% of full fare
US Airways - 10% of full fare
singapore- 10% of full fare (when flying Sq you could find a date showing a discounted fare)
ANA?
IB - 10% Full fare
Asiana- 10% of the adult mileage ticket, plus taxes & fees

On a side note, don't take it for granted that you can add your infant through the operating carrier. I never had success doing that.
I always managed to book infants with american on award flights booked with BA

Fuel surcharge is included in polished fare, I don't see how they would add it.

And IME with AA miles on ElAl they didn't add any fuel surcharge on top of the 10%.

IME and many others on this forum (see beginning of this thread),  AA charges approx. $800 for a RT economy USA-Australia ticket on QF.  When questioned about the ridiculous fees, they have stated that QF now charges a fuel surcharge on infant tickets.

i dont know based on what your information is, but QF charged me $146 for the infant RT JFK-MEL, which was even better of the fare american quoted me

then things must have changed since last summer.  I ended up booking my infant a seat bc that's what AA quoted me for the infant ticket and I called back several times... glad to hear.

No you would need 10% of the revenue fare.

If it's UA metal, you should book with Aeroplan and then only pay a flat fee for the infant. I think it's $100 for a J fare.
