AMEX Premium Rental Car Protection?

Can you describe more about this "AMEX Premium". I've heard about it but never had any experience with it. Which cards have it? Or which cards can it be added on to? Is it only for businesses? Does it cover additional drivers or just the cardholder? How about a spouse? What about 18-21?

You can add it to any AMEX card.
It costs $20/rental for $75K of primary CDW coverage and $25 for $100K of primary CDW coverage.
It think it only works for people authorized to drive the car.
