My Dr. gave me the card today but my insurance rejected the Celebrex. If anyone is interested let me know.
Not quite sure how it works I am sure it is like a coupon when you pick up the prescription.

By the way, if you're adventurous, I usually buy Celebrex (generic) from progressiverx.com It comes from India a couple weeks later. Cost is less than the copay from my insurance here. Used to buy generic from Israel, but this is cheaper. I've been buying the Indian generic for over two years, and it works fine. Every once in a while, I fill a prescription here (if I've run out, on vacation and forgot to bring, etc.)
