My wife has the US. Arrival plus and the rewards card. She got $25 on the us card. $0 on the arrival and $10 on the rewards .
I have a us and arrival plus. I only got on the us

You can request it right away. Or wait around and after a few months they will send it to you

I downgraded my US AIR about a month ago and they said they would still credit it to my account. So far I havent seen it yet. Hope it comes.

Ask the one who gets the check to scan it and email it to you so you can deposit it online/smartphone. (They send cards to IL, so maybe the'll agree to send the check to IL too)
Either way, can't you find anything to use your card for? It's no forex fees anyways.
