Got a letter in the mail this week stating updated terms as follows:

"The benefit is available to Consumer and Business Platinum Card and Centurion members only and applies when the Card member selects the "Global Entry" program. Other program applications including, but not limited to, NEXUS, SENTRI, and Privium are not eligible for the statement credit."

Dumb question; my infant would need a passport to be eligible for GE? Can I fill in the application without providing passport details?

It's only a few days from when you complete your application online until you get a notification by email that your application status has changed, and when you then log in, it says you've been conditionally approved, and you can set up an appointment for an interview.  You use the online scheduling tool to schedule the interview -- but can always go to JFK as a walk-in.  They are very nice there.
