This is getting well past what is appropriate for this forum. The rules are somewhat complex and should only be used by someone who is familiar with the ins and outs of their practical application anyhow. The short answer to both is usually but not always.

I'm asking for informative purposes, I don't plan on walking out of NYC on Shabbos anytime soon.

Does a map similar to this exist for NYC?

Also, while doing some Google research on this I came across this, which seems to indicate that Tel-Aviv is 4 cities for tchum Shabbos, though from this it seems like there are 4 eiruvin for carrying (as is obvious by lines not being squared), but tchum shabbos isn't addressed! How does that compare to NYC/Brooklyn/Manhattan etc.?
I've been waiting over 5 years with bated breath for someone to say that!

No such map exists regarding NYC to the best of my knowledge, partially because it is much more tedious than Beit Shemesh. It will go into Westchester and NJ and can cover quite a large area.

Regarding Tel Aviv they avoided the question of using ribua to attach and left it with this vagary.

My guess is that the red is the city border and the blue is the limits of the tchum. I have some questions on the map but am not familiar enough with the facts on the ground.

Not sure who they are, doesn't seem like the Tel-Aviv rabbanut, from which I saw no indication of said tchumin problem, though I didn't search extensively.
