AC: No carseats in lie-flat business class, carseats are allowed in non-lie flat business class.
CX: Front facing carseats allowed in First. Rear facing car seats not allowed in business or First. Front facing carseats not allowed in business.
DL: No carseats in business on A330, 777, 767 or 747 aircraft.
UA: No carseats in Global (3 cabin) First class. Allowed in business class and domestic first, though a seatbelt extended is needed when airbag seatbelts are present.

LH F-no go.

TK J-no issues.

LX F-no issues.

OS J-no issues.

RJ J-no issues.

UA J-no issues.

All DP's were with rear facing car seat.

So how does it work, you book a ticket for the infant, and if they don't allow it, you gate check car seat and then try to get refund for extra seat?

Good luck with that. Some airlines will carry 5-point restraints for premium cabin customers, but don't count on that either. If it's against the airline rules to use a carseat, I'd bring a portable restraint just in case (if the infant is big enough for one). Otherwise, pray for a bassinet.
