Highly advise to get an attorney. No 2 deals are the same.

You may be able to find something on Rocketlawyer or the like to familiarize yourself, but I agree with jackofall you need a lawyer to write one out for you.

At the very least, have a lawyer make you a form of agreement that works for you, and have him amend as needed for each deal.
