OK, the store manager of my Staples store who I know well enough for 5 + years is very helpful and knowledgeable, I purchased the Visa cards and NO REBATE FORM was printed, he logged in to his computer and printed for me rebate forms, with the offer number and rebate number. My brother went to a different Staples store, no rebate form printed, store manager told him he is unaware of a rebate offer and to call corporate.
