Alaska Mileage Plan- Does not issue infant tickets.
American - 10% of full fare
British Airways - Taxes and 10% of miles
Lufthansa/Miles & More - Taxes (Usually $17.50)
United - 10% of full fare
US Airways - 10% of full fare
singapore- 10% of full fare (when flying Sq you could find a date showing a discounted fare)

IB - 10% Full fare
Asiana- 10% of the adult mileage ticket, plus taxes & fees

My bad, if you add to a Y ticket then possibly they will let you bring him in, don't know for how long though..
Think they would make a big deal about it?

Im looking to book a UA one way flight EWR-TLV for 2 adults with an infant. I see that they charge a U.S. Federal Transportation Tax of $17.50 for the infant

What is that???

This looks like the right thread to share my last week experience with adding an infant ticket in my particular situation. The outbound adult ticket wasn't award however the idea is mostly the same.
Last Wednesday my wife flew JFK-TLV via FCO. This was a $223 ticket from the Priceline deal back in May (with the PRG-NRT leg). I needed to add an infant in lap. Now the ticket was sold by Priceline, issued by DL (starting 006), but no flights were actually on DL, as JFK-FCO-TLV was on AZ, and PRG-NRT on KLM. I called Priceline who told me they can't issue an infant ticket and that I should call DL. I called DL who told me they can't add an infant ticket because there is no record locator for DL since the flights are on AZ. I called AZ who told me they can't issue an infant ticket because... the adult ticket was issued by DL. So I kept calling around and got a different answer every time. AZ was (sometimes) able to issue the infant ticket (paper ticket at the airport) with prices varying between $230 and $350, more than the adult ticket. I then got a DL rep who said I could get a paper ticket at the airport for $39 (!). So we went to the airport and to the DL ticketing desk. Well no matter how hard they tried, and after going back and forth to AZ, they could not issue an infant ticket. At that point I had learned my lesson: adding an infant ticket can be a nightmare, and airlines rep have no clue what they are talking about.
But the solution we finally got was even stranger. The DL rep asked if the infant would also fly the PRG-NRT leg. I was a bit nervous but I said my wife is first coming back to JFK. For the way back she had a Flying Blue award ticket on DL but ticketed by AF (057). (I had called before about adding an infant ticket on the flight back and had been told that the DL desk in TLV would issue a paper ticket for $175). Now the DL ticketing desk in JFK happens to be shared with AF, so the DL rep went to the AF rep and explained the issue. Well the AF rep added the infant ticket both ways JFK-TLV-JFK to my wife's one way TLV-JFK ticket. Only $125 both ways. So the infant ticket was issued by AF (057) and travelling to TLV with an adult ticketed by DL (006), which I was always told is impossible.

Moral of the story, try with every airline involved to see where you can get the best deal.
