(It's not a hard rule. But most will keep closed because we don't say blessings and the like with it open, so it's the usual practice. I think it's the usual practice of most people though, no?)

I would say no. The reason being it lets you know that no one is in there. It is also an extra step.

My hand is not raised. I've got enough on my plate with my kids, B"H. However, both of my parents had pets growing up. My father says he had 78 cats, but not more than 11 at the same time. It all began when they let in a stray that was scratching on the door and meowing plaintively as a hurricane was blowing in... and the rest is history.

They smell. It is bad enough smelling after you go but before? 
A lot of people do put them in the bathroom.

That's why you put it in the bathroom. So it doesn't smell up the whole house.

We now have this issue with our rabbit who we currently keep indoors because of the cold/rain and the house is starting to smell a bit.
