You shouldn't have booked for the infant; you can always add the infant for free in the airport!
As has been hashed out many times, it may be with the 450 Avios not to have to arrive at the airport early to ensure that infant is added to the reservation
You can add the infant over the phone with AA. Save the 450 Avios and don't have to show up earlier.
Maybe yes. Maybe no. Happened to me more than once that stupid agents can't/won't add the infant. In fact, that's more common than finding an agent who will do it for you...
Completely understandable why someone would rather shell out 450 avios than having to call, possibly multiple times.
To each their own. But I've done it numerous times and each time, the first rep that I spoke to was able to add my daughter.

Both. But infant's name will only show up online when you pull up reservation if the trip is to Canada.
