There was a guy who had a gemach to lend phones that don't have lights and beeping for when your wife is in her ninth month so on the 1/7 chance you need to call on shabbos there are less melachos done.
I've asked several Rabbi's if that and Shabbos Taxi arrangement to avoid paying money are needed on the 1/7 chance. They all said that it's completely unnecessary. When in the situation you should minimize melacha as possible but you don't need to set up elaborate schemes and pay a membership to Shabbos taxi. You can have an envelope ready to go and the driver can take it from your house if you want to prepare...

Rabbi Forcheimer is of the opinion that you should minimize chilul shabbos to the greatest extent possible. others understand it's totally nidche and Shabbos would be disregarded. And there's a wide range between.
But, it's not always pikuach nefesh, and there's often plenty of time to take a taxi, and if there's time I think all would agree that you should minimize chilul shabbos and take a taxi, and use the phone less.
And Uber (maybe not in lakewood) is probably the least chilul shabbos and really quick

spanish ones should be fine. Don't know about Lakewood but in NY uber has Israeli drivers.

In Lakewood Hatzolah also has "shabbos goyim" who will drive you to the hospital if you don't need an ambulance.

Yes. I know 2 of them and they are actually EMT's as well.

They also probably know more Halacha about emergency situations then half of Lakewood.

County building on 4th St between Clifton and Lexington. (the building next to Pizza Plus) they will send out the form and help make sure it's done properly. Then just expedite it and you could have it in a week.

There is an extra fee for it.
I don't know / remember offhand. Ask them they will let you know. You could pay with a personal check there. (bring 2 or 3 just in case you need to make payments to multiple names)
