I sent a message to the "rewards" department via SM and received the following reply:
Dear Elite Flyer,
Thank you for contacting us regarding your Hyatt Credit
Card from Chase.
Our records indicate that your account was opened with an
offer to receive 1 Free Awards Night after the first
purchase and 1 free night after $1,000 in purchases within
93 days of the account open date. The offer to receive an
additional $75 statement credit was for customers who had
made a reservation online with Hyatt and opened the Chase
credit card account the same day. If you have met this
criteria, please provide us with the location of the Hyatt
Hotel so that we may confirm the booking date to compare
that with your application date.
If it is determined that a hotel booking was made on the
date of your application we will be happy to honor your
request for the $75 statement credit. If a hotel booking
was not made or was not done on the same date of the
application, we are unable to honor the request for the
statement credit.

So I wrote back again. Here is the correspondence:
" Dear ******,
It appears that you are mistaken as to the offer with
which I signed up for the Hyatt card. As a platinum
member, I received 2 nights after my first purchase on the
card. I have since achieved Diamond status with Hyatt.
For your reference, my most recent reservation was at the
Grand Hyatt Kauai. Additionally, please find attached a
copy of the offer which details the $75 statement credit.
There is no requirement that the application be made in
conjunction with a Hyatt Reservation. On either basis, I
would respectfully request that you accommodate this offer
for my recently opened card.
Thank you for your consideration,
Elite Flyer"
Here's what I got this morning:
"Dear Elite Flyer,
Thank you for contacting Chase about 9***.
I have received your recent inquiry, and I am happy to
advise you that I have credited the $75.00 annual fee. You
will see this adjustment reflecting on one of your next
two billing statements.
