we rented a car.
it was a pain in Milan because the area near the PH has traffic and no parking. Within Milan we only used it to get to restaurants. But since we were mostly out of Milan, it was great. We went with 2 kids so train wasn't an option.
Como has a lot to do but we allotted one day for it - we wanted to explore the whole region and were only there for 3 days.
That was also a benefit of the car - driving around Como and having the ability to stop in various places.

Thanks! I read that the driving can be scary due to sharp curves on mountains with narrow roads. Is this true or an exaggeration?

If you have a non-EU license, then it's required. The Italian law requires either the IDP or a sworn translation of your driver's license.
Most rental agencies do not enforce it, but some do, and if you get stopped by the police, you can get a hefty fine.

Is there anyone that ever actually got it? And is there any clear place where it states the amount of the fine? I've seen other places where it says it's a 250 euro fine...

It's called IDP -- P doesn't stand for a license but permit. It's more of a document that translates your license into English, French, Spanish, Russian, Chinese, German, Arabic, Italian, Swedish and Portuguese.

If you want to find travelers that actually got the fine, the best bet is to seek them out on TripAdvisor or lonely planet forums.
