Recommendations for reliable car rental 'excess coverage'

I'm renting in Italy, where they don't let you decline CDW by law. However, the included CDW has a very high deductible ($3k I think), which credit card CDW won't help for because I have some form of CDW from the car rental company. (Correct me if I'm wrong on this- does any card cover the excess/deductible when you can't decline CDW?)

Any recommendations of a reliable 'excess coverage' company? I see offers online for as low as $20 for a week-long rental, which is much cheaper than full coverage from the rental company, but I assume many of them might be an absolute pain in the neck to deal with if something happens. Any recommendations for a reliable one? Thanks.
