There's rarely one cause for a crash. The pilots had instructions about MCAS at this point and failed to properly carry them out. That's not to say Boeing isn't at fault, but that's also not to say the ET pilots are blameless.
Boeing basically did everything they could to avoid having pilots have to fly a simulator in the max with mcas. If pilots could verify flying the max without knowing about it, that's on Boeing/FAA.

Yikes. There was a "The Daily" podcast with a whistleblower from Boeing who was very adamant that they had a major issue with this
Life isn't about checking the boxes. Nobody cares.
