In the yeshivish crowd, I think there are three prevalent minhagim. 3 months before, 1 month before, and day of, with 1 month before being the most prevalent by far.

No other ceremony, but some people do go to a rosh yeshiva or rebbe to put on tefillin by the first time.

3 Months is Yekke.
IINM, Litvish is 1 month and many (most?) chassidim are day of. Because many in the "yeshivish crowd" come from chassidish, day of is fairly common. I don't think anyone other than lubavitch does 2 months.

Not going to ask about RT. I know that no-one does what Lubavitch has been doing since 5736.

I believe the common custom in Chabad us not to make a ברכה until you're sure you're doing it right.

Based on that standard it would be safe to assume that in today's (know it all) day and age one would start making a ברכה about 5 years prior to the BM (whether they have Tefillin or not).

Regarding RT; What I've seen is that most start putting them on after the Chassuna and some after the shuna rishona, with the exception of Nadvurna einiklech (Rosenbaums, Leifers etc.) who wear them even as bucherim. They have this awkward rectangular shaped Tefillin case since they don't need the larger ones for the Talis space.

How is it in Lubavitch?

I know that skver einiklach wear RT from bar mitzvah, viznitz einiklach do not.  The skvere einiklach also have the rectangular shaped teffilen bag. In my mind, it is a Lubavitcher teffilen bag because I saw it my Lubavitcher friends first.

Many breslevers also do RT from BM. Side point, bochurim in Tzanz wear tfilin Chol Hamoed.

Like it's מדינא דשו׳׳ע. Because the Sanz'e ruv zy"o said that Chassidus isn't for bocherim. So same goes for Bobov and to a smaller scale satmar too.

Printed in his name everywhere, very known. His saying was something like "חסידות פאר א בחור איז פילעווען אין אביי ורבא".

Obvious reason would be that a yid should start with fulfilling first his basic obligations in yiddishkeit, and only thereafter he should start focusing on reaching a higher דרגא בעבודת השם. Not that bocherim didn't learn ספרי חסידות at all or didn't practice other stuff like mikva etc. But the focus would be first for the basics, therefore Tefillin on Chol hamoed which is a מנהג החסידים ע׳׳פ קבלה and against the עיקר הדין ע׳׳פ שו׳׳ע, he believed that buchrim should stick to עיקר הדין ע׳׳פ שו׳׳ע.
