I'm new to sales force and would like to know where I can find the data from form.
City state etc.
We have an existing link and I want to add a location as well as remove and old one.

How was the form created? There are many ways to send data from a web form into SF. The most popular might be a web-to-lead form. You can check a form is a web-to-lead form by looking at the page source and searching for something like this:
