Procedure to book using these is to first make a reservation online and select:
1. Other forms of payment in payment information
2. Cash in drop down menu if available
3. Click purchase
A record locator will be created. Then call UA with your record locator and the gift card information. This way, your airfare is locked and CSR has a much easier time to retrieve your itinerary instead of creating a new one, saving a lot of time and possible fare increase.
You may have to babysit your reservation until it is ticketed. Apparently this is a manual process that some department within UA takes care of.
One certificate expires 6/18/2017, the other one expires 8/30/2017. It is possible that they can be extended for upto one year by ticketing a refundable ticket and then cancelling it: "If a purchase made using a Gift Certificate qualifies for a refund, the requested refund will be credited back in the form of an electronic travel certificate. The electronic travel certificate is valid for one year from the date when the electronic travel certificate is issued."
