LHR British Airways -
LHR BA Lounges - Kosher food is available at BA's lounges as follows:
T5/T3 Galleries Club: Bagel/panini and/or soup.
T5/T3 Galleries First: Main course, usually a choice of salmon, chicken stir fry or meat balls (sometimes changed to spaghetti bolognaise).
T5 Concorde Room (CCR): Same as First, but they will also bring soup from the Club lounge if asked, and can also do a plate of plain smoked salmon (with a slice of lemon) on request. Just make sure you stress that you ONLY want the smoked salmon, otherwise they will bring cooked cherry tomatoes or other stuff with it. They will also mix cocktails using Grey Goose vodka instead of the non-kosher Ciroc (Bloody Mary and Cosmopolitan) if asked. The staff in the CCR are very helpful.
Galleries First and the CCR also have a kosher breakfast option: omelette or 'frittata' with stuff which is supposed to make it look like a "full English Breakfast" - meaning it has a couple of rather poor Tivall sausages included. But the rest of the stuff is OK.
The arrivals lounge only carries the kosher breakfast.
Plain salted Kettle chips are kosher. If you want any for the flight ask at the lounge as they only carry two non-kosher flavored version on the aircraft. Also, in the CCR you need to ask at the bar for them in the packet otherwise they will (helpfully) open the bag into a pretty china bowl for you.
To get a kosher meal in the Galleries lounges you need to get one of the servers and ask them to check what kosher meals are available. In the Concorde Room (CCR) you go to the dining area reception desk and ask there, or you can ask at the bar, or indeed ask any of the staff who tend to float around. If you want soup in the CCR they may well tell you that they don't have kosher soups - they are not 100% wrong there because they don't store any of the kosher soup in their kitchen (which is shared with Galleries First). However, if you ask them to check with their supervisor whether they can bring it for you from the Galleries Club lounge they will always manage to arrange it.
LHR El Al contract lounge: BLUSH
LHR Virgin Atlantic ClubHouse, Hermolis meals and kosher wine
MAD Priority Pass T4S Neptuno had kosher meals on request (salami sandwiches, pasta salad, cake) with local Hechsher.
MUC KSML available with pre-request to Lufthansa.
SIN Singapore Airlines Private Room (Hermolis). KSML is available here only if requested 4 hours in advance per the waiter.
SYD Qantas First Lounge - available on advance request (Lewis KSML). Call CS and ask to be put through to the F lounge.
TLV Dan Lounges (not mehadrin/heter mechira).
TLV King David Lounge (mehadrin/shmita l'chumra).
YYZ KLM Lounge (terminal 3). Gradzinski Sandwiches are available before LY flights on Sunday, Tuesday and Thursday. And in Air Canada Signature lounge (terminal 1) hot kosher meals from Toronto Kosher are available but they need about half an hour to heat them up.

Not sure, I haven't been there yet.
Question is how high of an assurance level is required for bread? If you see it come out of an unsealed ZTB box is that good enough?
To be considered pas yisroel it would def need one chosam. As pas palter it would have the same rules as any other food item.

Update re BA in Heathrow - in Galleries (business class lounge) in T5 they now have leek and potato soup

As quite a regular there I can tell you that the Hermolis soups vary. It could be the leek and potato, or tomato, or mixed vegetables. I enjoy them all.

True. I meant that the Panini seems to be fixed at the moment - for a long time they had salmon bagels, now this instead. I thought it was good.

How are you able to ensure that they are really ZTB?
Only based on trust (and that I recognize how pastries looked in ZTB bakery). I was there in the evening, pastries and bread was on plates on the buffet. Kitchen manager showed me the receipt/delivery notice for all the pastries received that day and I confirmed it was from ZTB.

Things are very different now than in the times of the Mordechai. This is discussed in poskim. Bread can have all sorts of ingredients which they did not use then. Release agents are used which were not used then. There are breads which may have shared equipment with the plain breads etc.
I mentioned permitting legislation. Fundamentally there is no difference in terms of whether you should trust say your French baguet today and back then, it is simply a matter of trust. Secondly, there certainly always was the issue of using the ovens for other items. This did not bother the rishonim for various reasons. The main difference is that 'we' are much frummer then the Mordechai or most Ashkenazi Jews leading into the 19th century.

This is simply untrue and ignorant. This is all discusses extensively in the poskim, but that wouldn't fit with the narrative you are trying to have.

Only in F lounges, and then only if flying CX (not if accessing due to F flight on other airline, or due to status)

What's the story with the King David lounge in Heathrow? Is there kosher food reliable?
