You can make up.
Best is, just fill in your name in the "business name" field.
And use your SSN as your Tax-ID #.

When I got my business CC I didn't know what to write for Tax-ID so I just put 000000000, when asked for SSN I gave my real one.
I got approved.

My guess is an analyst switched it (or it got switched automatically) to your SSN.

which cc company?

Just write your SSN instead of the tax-ID, any CC issuer.
