Balance Transfer Cards?

I'm looking to transfer ~$20k to a 0 APR Intro card.
What are the best cards for balance transfers?
Any of them earn bonus points or rewards or miles on BT?

Any with no fees?

Keep in mind that you might be charged 3%/5% of the amount transferred
