U.S. Federal Transportation Tax: 32.60   
U.S. APHIS User Fee:                 5.00   
September 11th Security Fee:   2.50   
Israel Departure Tax:                24.85   
Fuel Surcharge:                         514.00   
U.S. Passenger Facility Charge:   4.50   
Per Person Total:                        815.95  USD

I thought it was pretty funny that the airfare charge is only $220.

Fuel surcharge is technically part of the fare, they just decided to separate it into two charges. So it's really Fare A $220 + Fare B $514 = Total Fare $734
