Because chalav stam is not a chumrah that's been around for hundreds of years.  Rather it is not an issue at all.  Since Rav Moshe does mention a makom lehachmir,  I have actually heard rabbonim include it as well,  in the general spirit of hiddur.

Because pas akum is muttar midina, and as a hiddur it is brought on the seforim to be machmir in the aseres yemei teshuva. Chalav Akum is assur midina always (with the exemption of the unique heter of R"Moshe which was never intended for Baalei Nefesh), so there is no reason for the poskim to  mention aseres yemei teshuva.
This is a ludicrous misconstrued way of presenting a halacha.  According to Rav Moshe,  and those he quoted,  it's also muttar midina.  And therefore OP'S question is perfectly valid for the vast majority of USA Orthodoxy that eat chalav stam.

Why do you expect the rema to talk about something that wouldn't exist until 500 years later? Isn't that the straightforward answer?

I guess we're looking at the same point in two very different ways.

Most definitely. You're starting from איסור דאורייתא and I'm starting from מותר לכתחילה.
  I feel that since this was the position of the Lithuanian (at least) posek hador,  and is the accepted opinion to most of the non chassidic world it should be considered with more gravitas.

When Reb Reuven Feinstein shlit"a was asked what a Baal Nefesh is his response was "Not us".
A baal nefesh would probably be defined as one who foregoes most worldly pleasures. Not someone who goes crazy over Izzy's smoked ribs. (myself included)

Answer: poskim bring down that you should be machmir on things that are explicitly chumros (like pas yisroel) and not on things which have a tzad halacha (like yoshon). Which category does cholov yisroel fit in?
