I think that this is a highly subjective question that can't really be asked in general terms. A couple of examples:

- What prompted the mistake? Was it preventable by the "employee", or due to circumstances that weren't entirely in the employee's control. For example - if the employer neglected to mention a certain requirement of the project and the
- If this was for a time-estimated job (i.e. this job would take about 40 hours of work), does it significantly alter the scope of the work? (For exampe, I told the client it's 40 hours of work, and this mistake takes it from 34 to 37 hours - the client is still paying less that the agreed estimate).

Forgetfulness. Most of the time something was done one way. This time the employer said to do it slightly different. But employee forgot.

If I understand correctly a woman is  mechyuv in Kiddush but not in davning, so meaning if she wants to eat she would need to make kiddush before she eats almost anything whether or not she davned yet.
