There is no Tzaar balei chaim to kill a specie, even for fun, only when torturing it alive. Eating it alive might me biklal killing it, not touring it.
Eating it alive is baal tishaktzu, regardless of whether it's also tzaar baalei chaim. That much at least is explicit in the Tosfos quoted above.

There appear to be poskim who say there's no tzaar baalei chaim by fish (https://judaism.stackexchange.com/questions/1864/tzar-balay-chaim-for-fish 2nd answer), and none of them bring this Tosfos as a mekor. In fact, you mentioned that the Chinuch holds it does apply to fish, but at least one of the poskim who hold it doesn't bring a proof from the Chinuch.

Bottom line, it does appear to be a machlokes haposkim, but I'm not sure either Tosfos or the Chinuch are anywhere near explicit on the issue.

Correct. It's Lafukei aiver min h'chai which doesn't apply to fish.

First off, there is no Issur of Baal Tashchis at all if it is for a good purpose, even if it is only Assur mishum minhag and certainly if it is an issur gamur (whether D'oraysah or D'rabanan)
Now, when it comes to the other 2 (in certain situations of a choleh or the like), if the light is incandescent/halogen, that is the worse issur. If not, they would be equal, BUT PLEASE ASK YOUR LOR IF IT'S L'MAASEH

Igros Moshe O"C 2 - 105R'
Moshe says that as a Horoas Sha'a, people who will be unable to say slichos in the proper times  with a minyan, can say it before chatzos and while it doesn't have the Havtacha of the יג מידות that wont be returned empty-handed, it does have the chashivus of a regular tefilla, and therefore it's better that they do this and not lose the opportunity that slichos provides.

Baal Tashich is certainly not a consideration in this circumstance.

If Bal Tashchis would allow one to be mechallel shabbos, we would have no more shabbos... every important work call etc.
