My thoughts are that this may be a delayed reaction to the virus in children with old, undiagnosed Covid infections from the past 4-12 weeks. I pray I'm wrong.
It is highly suspected that this condition is a late-stage inflammatory response to COVID-19 illness. Many, but not all, children with this condition tested positive for COVID-19. Even in those children who did not test positive, COVID-19 illness is still suspected because the tests themselves have had varied accuracy. One suspected possibility is that the antibodies the affected children are making to the COVID-19 virus are creating an immune reaction in the child's body. No one knows for sure yet.

I understood that to mean they are testing positive now. I'm wondering about initial exposure, since the timeline seems to be a couple of months behind the spike in adult positives.

There are other countries that are months ahead of us on the timeline. Wouldn't we be seeing it there?
What countries are months ahead? They are a month ahead, and it’s possible the link wasn’t made yet. Hopefully this doesn’t happen but there’s no denying there’s been a significant spike in these cases.. NYC, which typically sees 53 cases a year, now has 73 cases in the hospital at once. Most COVID or antibody positive.
