Link to form that needs to be filled out within 48 hours of flight to Israel

Accordingly, the FINAL criteria For entering Israel are as follows - you must meet ONE of these conditions in order to enter Israel:

1. Vaccinated within 180 days of the trip with either a second vaccine or a booster and it must be at least 14 days after the shot.

2. Recovered within 180 days of the trip with DIGITAL proof of a positive PCR result and more than 11 days have passed since the positive result. This type of proof only exists in the EU. The UK will likely have it as well.

3. Recovered anytime – even more than 180 days before the trip - with DIGITAL proof of positive PCR result AND vaccinated with at least one dose within 180 days of the trip. Again at the moment the digital proof only exists in the EU and possibly the UK shortly.

Your vaccination or recovery CANNOT expire while in Israel, which means you may not be in Israel more than 180 days after your vaccine or recovery have expired.

CONSEQUENCES FOR VIOLATING THE TERMS OF ENTRY: VIOLATION OF ANY OF THE ABOVE CRITERIA CAN MEAN SANCTIONS INCLUDING BARRING YOUR ENTRY TO ISRAEL FOR 5 YEARS.

Yes. Your spouse doesn’t need to be traveling with you.

AA - moved their JFK flight later - so now lands at 6:10pm - so not possible to make the Shamir antibody test on day of landing - ends at 6pm. Anyone have an experience with getting a test after 6pm with quick turnaround?

Also - anyone with recent experience - for Motzei Shabbos flight back to US - is the only option to test on Thursday (Friday too risky with lab turnaround before flight) ?

Last question - what cell number is used to track quarantine? - any issue using israeli cell of a family member in israel for multiple people? Can you use a US number?

Green Pass (or other alternatives, of which I am unaware) is required in order to enter terminal. It isn't requested at any other point (negative COVID test is requested at check-in) AFAIK barcode isn't scanned. When I entered ID number was compared to whatever is shown on passport. Not too difficult to edit with a different ID number, and it works for that purpose.
I've been waiting over 5 years with bated breath for someone to say that!

It doesn't make much of a difference. I would suggest any test that actually shows a number rather than just positive or negative. This is only for your own knowledge, you will need an Israeli test in order to exit quarantine.

Vaccine makes no difference. Covid test is required by the US in order to board flight.

One can definitely test on Sunday in Israel. Cheapest that I'm aware of is at the airport, though if you're flying Sunday night you might want to get your test rather early on Sunday (as a backup, just in case it doesn't come back on time, just edit the date on the test you took upon arrival in Israel).
I've been waiting over 5 years with bated breath for someone to say that!

also, for quarantine do they gather the info from here D. Completely filled out 'personal details' declaration and quarantine declaration, click here to download or from the הצהרה you fill out 24 prior to flying?

FWIW my Canadian father in-law applied last week via email to Toronto Consulate with his son's U.S. Passport, Student Visa and Israeli Driver License...and he received Ishur a few days later without any questions!!! Meanwhile I applied via email to NY consulate with my brothers documents (hes a permanent Residence) and they denied it as he never got an official Israeli ID Card! WTF

Just my gut feeling to know how far away from the threshold you are.

HOWEVER, I was just asked by someone for advice. I told him that I can't give advice, I can only share my experiences and observations. He took two antibody tests in NY about 8 days apart.

On 4/13 he took a test that was sent to ACCU Reference Medical Lab in Linden NJ that showed the following results:
COVID-19 IgG ANTIBODY 5.6 (Reactive)
COVID-19 IgM SERUM 2.1 (Reactive)
(no test name on results printout)

On 4/21 he took a test that was sent to Empire City Laboratories (ECL) in Boro Park that showed the following results:
SARS-VoV-2 IGG QNT 11.3 (Negative) where reference is 0-15.0 for Negative.
Test performed Diasorin Liaison XL

To be on the safe side and exit quarantine in Israel he decided to take one dose on the Pfizer vaccine (though my guess is that he would show positive for antibodies in Israel even without it).
I've been waiting over 5 years with bated breath for someone to say that!
