Not recommended to enter false info on a government document.

Is someone legally required to fill out the form? Not sure they can legally force this...

I am still not clear about the official quarantine rules for those coming from Israel to the USA. My family member (not a US citizen) had to cancel his flight when he had a stop over from Israel to the USA. He had to rebook a nonstop flight from Israel to the USA.

Where can I read up on the official rules?

New York Governor Andrew Cuomo says New Jersey has qualified for New York's quarantine list, along with 41 other states and territories. But says there's "no practical way" to enforce a quarantine with the two states, calls for all non essential travel to be avoided.

Flew into JFK last week and didn't see any national guard (but I was coming from a state that didn't require quarantine).

I think it depends on the flight and airline. Every time I come in with AA they're there (most recently last week). Any other airline nothing doing. Just my personal experience.

UPDATED NY COVID TRAVEL RULES: New York is doing away with its quarantine travel advisory list and replacing it with a blanket testing requirement. Instead, anyone traveling to New York from out of state must have proof of a negative test dated within three days of arriving in the state. Travelers must also quarantine for at least three days upon arrival and get another COVID-19 test on the fourth day.

I doubt enforcement will go up, but this is more reasonable, and should be implemented globally.
