That letter doesn’t say anything about what’s happening in Lakewood now. And that silly wordplay at then end about poly- vs. mono- is misleading and unscientific. The vaccines are also likely to be less effective against variants, but the reason to prefer vaccines over monoclonal antibodies is because being sick isn’t a good thing and should be avoided when possible, not because vaccines are more effective.
Doesn’t mean this doctor has any proof/evidence that it’s a variant that’s prevalent in Lakewood. Your explanation on E484K seems to refer to what’s described here:https://www.reuters.com/article/us-health-coronavirus-science-idUSKBN2AM2GQ
From a Lakewood Dr.’s pre-Purim COVID email:

Does anyone know how teachers can get the vax in Lakewood?
Private school teacher of public school teacher?
I'm still trying to find out how to get a vaccine for an eligible healthcare worker....
That's insane caretaker for an elderly person?
No. Just a vendor that works in assisted living facilities. For now, they're avoiding being on site as much as possible to avoid bringing in any virus.
Said facilities (or their employer) can't help? Are they not allowed to drive to NYC to get one at a federal vaccination site here?
[Expectant women] are definitely at a higher risk at this point. In recent weeks, we have come across hundreds of cases that resulted in miscarriages, and sometimes death of the young mothers themselves.
