Car Rental- Chase points

Ii am renting a car using Chase UR Points from a sapphire card. Will I be covered by the insurance on the card even though I am not charging anything to the card?

If I rent a car through the Chase Sapphire portal (I have the reserve) and I decide to get an upgrade when I actually go and rent the car, will that be a separate purchase? Does it go through my Chase points?

Chase is not charging me a underage fee when booking a rental for someone under 25. Is that going to be charged seperately at location?
