1 if i don't have another insurance is there any benefit in getting this
1-small benefit, not really much (if the rental company decide to be nasty about it, you're safer like this)

Im missing something.
if you dont have other insurance and you dont have this insurance, you'll be out of pocket for all the repair replacement expenses if G-d forbid something were to happen to the car?
Why is there no benefit?

almost all CC give you LDW secondary so if u don't have insurance the CC will pay for it.

however, I'm relatively sure that the coverage provided by CCs for car rental is only for contracts that do not exceed 15 days, so if you're renting for longer than that, you'd probably be in deep water.  This policy however is good for rentals of upto 42 days (in almost all states) and you cant usually even rent for more than 30 days consecutively.

some CC 15 day max, some 30. if you're going to do longer than that but less than 40+, you're right, you'll need amex premium coverege

tried and tested, rented a car with the premium insurance from AMEX and when I returned the car and the employee checked it and there were no problems, I then paid with another card, and never got charged a thing from AMEX.

Also for those with billing addresses in CA the charge is only $17.95

From my recollection, Visa and Mastercard both provide secondary CDW for 15 days max with a $50,000 limit and AMEX provides secondary CDW for 30 days, also up to $50,000.
AMEX's insurance (I think Visa's too), will not cover full size SUV's and pickup trucks (AMEX specifies Chevrolet/GMC Tahoe, Yukon and Suburban, and more) I think Mastercard will cover Suburbans etc. put not pickup trucks.
There are occasional Visa/Mastercards that have other insurance which have other terms, like OPP that has primary or Citi AAdvantage world MC (30 days).

With AMEX's premium insurance, the advantages are:
1) Primary coverage
2) $100,000 limit
3) Cover more car categories, including full size SUV and pickup trucks
4) Coverage up to 42 days

just looked into it there is a "Up to $15,000 for excess medical expenses per person." is that included in most credit cards secondary insurance?

standard coverage is for AU's as well and the premium coverage can be activated on AU's while not on the primary card but if you activate it on the primary it automatically activates the AU's (I guess you could probably get them to cancel the AU's if you want just the primary)

I only saw the one option since I was logged in on AMEX's website and I have a CA billing address.

It should be enough unless you are renting a car that has a replacement value of more than $75,000

What about damage to other('s) vehicle/property? What does someone with assets do to protect/insure themselves in such a situation?

Isn't that Liability? No ammount of CC coverage would cover that. Either you rely on your own insurance coverage or take out LI

Can't help you with the rental stuff but collision insurance covers damage to your car while liability insurance will cover damages to others that you are liable for
