In Lakewood it was name phone number and email address

Is not on where you live because people that don’t live in us and test positive are not reported as foreigners.
I think is more likely of where are you testing, which can pose an interesting investigation on the “hot” spots.....
Maybe someone with first hand knowledge can chime in
