This is really scary stuff. Are there no humans operating the deicing to give the all-clear? And if they did while the wings were obviously not properly de-iced, those people should be charged with the most convictable offense possible. They literally were ready to endanger the lives of everyone on the plane!
From what I understand, the de-icing people give a datasheet to the pilots after a de-icing, which includes how long they can be on the ground for before they have to de-ice again, etc.
If they gave that datasheet to the pilots with false info, then it's pretty clear cut where the blame is.
