Maybe I misunderstood this So you are saying that if the first responder bumps into the terrorist forest he will work on him over the victim a few steps away who will die of left unattended?
As I said, he might make his own decision, I don't think there is a rule that would let him discriminate. In that setting his decision is what counts, but when the situation is in a hospital like you mentioned earlier, I believe the call would not be allowed to be based on anything other than age of chance of survival.
