Filed early April- received state refund pretty quickly, nothing from fed- status is still processing. Accountant says nothing to do but wait.
That's a long time to be pending. Did you call the IRS and ask about it?
Did you have unemployment pay.
Call at exactly 7:00 am.
Sometimes you need to get someone to give it a push to the next stage. It's also possible they sent you a letter for you to respond to but it got lost in the mail.

Spoke with my accountant, he said a lot of people got there refund in the last two weeks and usually a higher amount then expected.

Here's a data point:

Filed taxes in late February, got the refund soon after. Then I became due to an increased refund due to the American Rescue Act (making part of unemployment untaxable). That refund came through today via Direct Deposit.

Be aware, you would need to report this interest as interest income on your 2021 tax return.

Worst case they’ll send you a bill, even if there’s interest and penalties on it, that’s a percentage so on such a small amount (whatever part of the $40 is taxable), so it won’t be much.

It might or might not be an audit flag, but it’s hard to believe something so minor would be.
