Aeroplan - Flat Fee per booking $50 (coach), $100 (Business), $125 (First) All the these prices are CAD. Plus tax.  - or can use miles (bus was 10k so assume @ 100/$1 for any)
Alaska Mileage Plan- Does not issue infant tickets.
American - 10% of full fare
British Airways - Taxes and 10% of miles
Lufthansa/Miles & More - Taxes (Usually $17.50)
United - 10% of full fare
US Airways - 10% of full fare
singapore- 10% of full fare (when flying Sq you could find a date showing a discounted fare)
ANA?
IB - 10% Full fare
Asiana- 10% of the adult mileage ticket, plus taxes & fees

Also if I am reading correctly LH gives an infant a free bag besides for the carriage which is pretty sweet.

I don't mind buying another award seat for infant, but will I then need to bring a car seat? How about if I don't buy a seat, do all airlines offer a basinett in business? How do I find out

Anyone with an upcoming business/first flight want to try and see if this works with an infant coach ticket attached?

I'm willing to put in $100 to start a pot to pay the volunteer in case the worst case scenario happens and they catch on and insist on you paying 10% of the full business/first fare at the gate.  Anyone want to contribute or volunteer?  You could save yourself a small fortune!
I did this last week on UA, I booked 2 ow j mileage tickets and made the infant a ticket in economy via a ta, and when they went to checkin it took a phone call from the checkin supervisor to the united Helpdesk in order to attach the infant to the reservation but they attached it np and checked the baby in with no mention of economy vs business.(I think that they can attach it over the phone also beforehand) ended up costing me $77 instead of $770 :)

LH F rezzie, booked with UA, infant booked by UA in Y (by mistake).  At check-in, LH couldn't find infant ticket, forced to spend a considerable amount of time on the phone with UA to fix it, but the flight closed while trying to get it fixed.  Ultimately, not sure what would have happened, as the pax was rebooked on a later flight, all in J. But not simple.

You made an INF ticket with TA but didn't have them attach to the reservation?  Obvious you were going to have issues.

Assuming UA metal?

I now give the honor to the such booking requests the client himself (if they are only buying INF tickets by me) to attach the ticket. TA in this case has to call the airline posing as the pax to do it and no reason I should waste a few hours to get it done. Coming from there.

I booked it on the day so I wouldn't need to pay for the infant ticket if it didn't work out  and yes it was ua metal

if the baby would have been attached by lh it most likely wouldn't have been an issue, the issue was most likely the communication between SHARES and Amadeus
