Aeroplan - Flat Fee per booking $50 (coach), $100 (Business), $125 (First) All the these prices are CAD. Plus tax.  - or can use miles (bus was 10k so assume @ 100/$1 for any)
Alaska Mileage Plan- Does not issue infant tickets.
American - 10% of full fare
British Airways - Taxes and 10% of miles
Lufthansa/Miles & More - Taxes (Usually $17.50)
United - 10% of full fare
US Airways - 10% of full fare
singapore- 10% of full fare (when flying Sq you could find a date showing a discounted fare)
ANA?
IB - 10% Full fare
Asiana- 10% of the adult mileage ticket, plus taxes & fees

Ask the UA agent to create a ghost ticket, then add the infant on it and then put the infant on your ticket.

Fifth HUCA with UA worked! Still waiting for the email, but got confirmation code and cc billed.
If they made you a ghost ticket to book it, and then "attached" it I would advise you keep track and make sure your infant ticket does not disappear after the ghost is cancelled.

The infant is ticketed.
They (UA) created a separate PNR for the infant.

Can i ask when you are flying? UA only ticketed my infant a few days before the flight, and I'm wondering if that is across the board

Ticketed about 30 hours before the flight, but only purchased adult ticket about 5 days out.

Just another data point on another award ticket using SQ miles on Star Alliance metal (in this case a mix of LX and UA with UA as overwater carrier. SQ again could not book an infant ticket because they could not find a base fare. UA agent was able to do it pretty easily once I insisted that SQ could in no way do it.

So, it seems SQ cannot issue Star Alliance infant tickets for their award tickets (unless perhaps SQ is part of the itinerary)...

Any data points of successful SQ issued infant tickets for *A flights?

Were they able to actually issue it? Did it include any SQ legs?
I am guessing that it depends on whether their ticketing office can sell a cash ticket. Maybe some routes/airlines are in their system and others aren't.

That's true in theory.
I'm looking for data points on issued tickets. Did they quote you or actually issue the ticket?
And when was this?

I don't know about your particular issue but it's free to cancel the infant with Aeroplan. Cancel the infant, get refunded and book it again. If there's another adult with you, try to put the infant under the 2nd adult's ticket.

Did you get an email from Aeroplan with the infant eticket information?

I’ve seen where they added the infant name, but didn’t actually process the ticket (causing issues).

Ticket number doesn't always transmit. If you have a ticket number provide that to UA they should attach

So I called back United again just to see if anything changed. The rep told me that she sees the lap infant on reservation but she mentioned it was booked differently than United would book it. So infant is not attached to a specific adult but he’s on the reservation. Will that be a problem that he’s not attached to an adult?

They both claim to charge 10% miles [cxcan be  up 2 25%] the question is what taxes and fees are added for the infant.
Always praying for delayed baggage.
