Aeroplan - Flat Fee per booking $50 (coach), $100 (Business), $125 (First) All the these prices are CAD. Plus tax.  - or can use miles (bus was 10k so assume @ 100/$1 for any)
Alaska Mileage Plan- Does not issue infant tickets.
American - 10% of full fare
British Airways - Taxes and 10% of miles
Lufthansa/Miles & More - Taxes (Usually $17.50)
United - 10% of full fare
US Airways - 10% of full fare
singapore- 10% of full fare (when flying Sq you could find a date showing a discounted fare)
ANA?
IB - 10% Full fare
Asiana- 10% of the adult mileage ticket, plus taxes & fees

Planning a trip with my wife to Spain with my wife and baby using United miles. Right now only TAP has availability and I see they're only charging $300 for the infant. My plan is to try to switch over to United if it opens up, but I know see United charges $571 for the infant. If I'm succesful with a switch over in carrier down the line, will United charge me the difference in taxes as well or since taxes already paid up for infant, they'll let me go? Anyone have any experience? Thanks

BA charges YQ for the actual ticket as well, so QF on BA is not very ideal.
Best way is to add through a TA

No. he can book the infant separately, which is fine.
Not all TA's will do this for you obviously, but some will. PM WhyAich, he'll assist you if he's not too busy with his newbie.

I just bumped into this thread so I will add a minor tidbit of info.

I recently flew EWR-ZRH-TLV/TLV-ZRH-FRA-EWR in a combination of UA/LX/LH on business reward tickets.  I bought the ticket for myself with UA miles and for my wife with US miles

UA added my baby for $356.  US had wanted nearly $1K to add the baby.

When I tried making changes to the ticket, still within the no charge to change rewards period with UA, I was told that the infant ticket was like a revenue ticket and subject to a $250 change fee
