Google Forms Form populate

Does Google/Microsoft/Jot Form have feature where on the top you can select what form you want and then it will fill in the form. I want one link to a form for two things.


For example I have two sports league, a football league and a baseball league. I want to use one link and on the top of the form I want the people to select if there are filling out the form for the football league or baseball league, if they are filling out form for the baseball league, when they select baseball I want certain questions to populate if they are filling out the questions for football I want certain questions to populate.

The feature you are looking for will probably be called conditional field visibility or something like that.
Unless you want the results of the two "forms" to go to two separate places...  in which case you will need 2 actual forms

Unless you want the results of the two "forms" to go to two separate places...  in which case you will need 2 actual forms
I know jotform has this. based on the first question (baseball/football) it can hide or show the appropriate fields.

I've done it with GravityForms on a Wordpress site too

AirTable has a free form service that may be of interest.
