I can't access the link - does the test have to be PCR, or is an antigen test sufficient?

The link doesn’t say. Many places don’t have US-approved antigen testing widely available, nor cheap PCR testing where results arrive within 24 hours…

I assume positive test within 90 days and document of recovery would still work too?
