The minimum spend is $4k in 3 mo.

This is the highest that I have observed. My questions are:
I am actually planning to start my own business. Yet, since this entity has not been formed yet, how should I fill out the application which asks for information such as annual gross revenue, ownership format, business name,...?

Even if you don't have a business, you can apply for business cards using your SSN under EIN number and then just fill it out regularly. But it obviously helps to have a registered business.
