That is because it is understood that there are functions and מצבים that require this or the whole ענין of משפט and מלחמה will be בטל. Therefore one who is fulfilling this function -or is a doctor- cannot operate according to the rules of לא תעמוד על דם רעך and חייך קודמים that apply to everyone else. He is in fact obligated to put his life on the line. )

It is a fundamental mistake to conflate “risk taking” with an outright act of murder. To the former there are plenty of sources for war having different parameters, for the latter there are none.
