You have to chose the right options to get the card.  What worked for me was Richmond VA for location (don't worry about the card, they email it), none of the above for industry, say you went into the store (not drive-thru) and during the hours of 7-9am.  If you get disqualified at any point, open a new incognito browser and try again.
