Do you ask this about every hierarchy that exists in virtually every industry?

You don't usually have a sign up saying that you have an employee making sure that the other one does his job. Having a yotzei vinichnas when you have a mashgiach tmidi basically says we don't trust the mashgiach. With that being said ,its just worded poorly. They probably mean to say that there is a mashgiach tmidi along with extra oversight and inspection by one of the certifying rabbis.

Lol, every single business or store has that whether there is a sign or not. Every person on this forum understood exactly what the sign meant.

Do you run a business? Do you just hire people and hope they do their jobs without any oversight whatsoever? Oversight doesn't = a lack of trust.

Besides, the yotze v'nichnas is not just overseeing the mashgiach, they serve an extra supervisory role that is different from the temidi.

This is standard practice not just in every hashgacha (or at least any that is worth its salt).

I agree ,just I think that yotzei vinichnas (by a mashgiach) usually means as opposed to temidi and when used together sounds like more than oversight and is therefore a poor choice of wording.

The mashgiach tmidi may not be a dedicated mashgiach but a jewish employee of the store who the hashgocha is relying on hence the need for a real mashgiach being יוצא ונכנס. I write this not with knowledge of the specific situation but as a possibility knowing that such a system is sometimes used by hashgochos.

I would think the latte itself may be DE as well. (I eat chalav stam, but if one doesnt this should be problematic).

It probably is a issue as they can clean the equipment together with non Cholov yisroel equipment or worse if they sell other food…

What I heard from R Kuber ~6 months ago: He had just went to their offices and went through the hashgacho and said that he found it to be very good. Apparently everything they do is special production.
