My top 5 would go something like this:

Asian (Buenos Aires).  Delicious free cocktail when you arrive (try their caiparinha and daquiri as well!) along with breadsticks, bread and a number of very good dips, outrageously good appetizers like nothing you've ever had before (Thai Beef Salad, Beef Rolls and Chicken Rolls) and mouthwatering Roasted Beef Entree with succulent roasted potatoes and fries.  And you can order it all without breaking the bank.
La Seine (Beverly Hills).  Out of this world sushi, perfect steak (ask for a variety of sauces on the side), and a warm Lemon Tart is a must.
Tierra Sur (Oxnard, CA).  A menu that constantly changes, is completely different for lunch and dinner and has flavors that you've simply never tasted before.
Le Marais (Manhattan).  Pepper Steak is about as perfect of a steak as you can ask for, the salad, mashed potatoes or fries that come with it are excellent, and the hot chocolate cake with Ice Cream is the best I've ever had. 
La Gondola (Beverly Hills).  You simply can't miss the Avocado Egg Rolls or the Asian Spring Rolls.  BBQ Ribs and Chai Steak are generously sized and awesome.  Chocolate Chip Souflee with Ice Cream is top notch.  Lunchtime special of a Gondola Burger and a soup, fries, or homemade pasta for just $10 can't be beat.

Other random kosher eateries that come to my mind in no order whatsoever. 
These may not be the best in the world, though at least some things they make are excellent.

Basil (Crown Heights)
Ladino Tapas Bar  (Manhattan)
Shallot's (Chicago)
Darjeeling (Paris)
Jerusalem Kosher Deli (Honolulu)
Falafel In Paradise (Kailua-Kona)
Rare Steakhouse (Miami Beach)


East Coast:
Clubhouse Cafe (Manhattan): Temporarily closed, but awesome burgers and fries and great caiparinhas.
Benny's Pizza, Boro Park Eggplant Pizza, Felafel Pizza, Pasta Pizza...carb loaded slices of heaven.
Gombos' (Crown Heights) Chocolate kokush is simply perfect.
New Kosher Special (Elizabeth, NJ): Never been inside (and I'm told that's a good thing) but free and easy delivery to Newark airport and the Sesame Chicken was cheap and really good.
Umami (Baltimore): Pad Thai Chicken can't be missed.
David Chu's (Baltimore): Probably the best kosher chinese you'll find.
Shemtov's Pizza (Miami Beach): Excellent Pizza, pizza fries, and don't miss the cinnamon macchiato.
Jona's Pizza (Miami Beach): Unimpressed by the Pizza, but the grilled sandwiches were incredible.
Schakolad (Miami Beach): Hands down some of the best milk chocolate and caramel I've ever had.  And don't miss the Gelato.
Harbour Grill (Surfside, FL): The steaks are good, but the sushi is excellent.  Service OTOH is not good, an 18% tip is stealthily added to your bill and then they try to get you to tip again on top of that and beware the frozen onion rings that they will say are awesome.
Skoops (North Miami Beach): French Vanilla Skoopachino is awesome.

Midwest:
Ken's Diner (Chicago) A classic.
Illinois Nut (Chicago) Don't miss the parve caramel bombs or english toffee.
Unger's (Cleveland): The best glaze, cinnamon, and custard donuts around.
Contempo Cuisine (Cleveland): Ambiance is not good, but the pepper steak and garilc mashed potatoes can be wicked at times.
Kinneret  (Cleveland): Biased, but their plain pizza, spicy fries, and homemade fried cauliflower and onion rings are all very good.
Bagel Shoppe (Cleveland): Good cookies, great jalapeno and garlic bagels.
Sarah's Deli (Detroit): Bubba's works burget with pastrami and homemade skins with ranch dressing.

West Coast:
Fish Grill Beverly (Los Angeles) : Angel hair salmon pasta with fries are delicious, though I was disappointed with them at the Pico location.
Milk and Honey (Los Angeles): An excellent Eggplant Parmesan (ask for extra sauce and ricotta cheese) and very good brown bread and dip.
YoGo (Los Angeles): As good of a cholov yisroel yogurt bar as any that I've seen.
Cafe Elite (Los Angeles): The best cake donuts around, get the crumb ring.
Schwarzt Bakery (Los Angeles) Crumb topping cheese danish, kit kats, and caramel cake.
Coffee Bean & Tea Leaf, Beverly (Los Angeles): Iced Mocha and iced Vanilla coffee.
Pico Kosher Deli (Los Angeles): A hot pastrami stacked high with cole slaw and homemade onion rings.
Pizza Nosh (Agoura Hills, CA): Excellent pizza and eggplant parm.

North of the border:
Cafe Sheli (Toronto): Very good french onion soup and eggplant parm.
Omni java and jewels (Toronto): Excellent cheesecake and great coffee drinks.
Second Cup (Tonronto): The cholov yisroel frozen coffee drink is great.
Yakimono (Montreal): Excellent sushi.
Mount Royal Bagels (Montreal): Incredible wood-oven bagels .
Ernie and Ellie's (Montreal): Go for the chinese tasting menu.
Pizza Pita (Montreal): Have to hit up this place in Montreal.
Cheskie’s Bakery (Montreal): The best dairy baked goods you'll find anywhere.

Abroad:
JCC Hong Kong: Their Sunday night buffet can't be missed.
Katzy's (Sydney): One of the few decent kosher eateries down under.  Don't miss the beef ribs.
Pitzman (Paris): Similar to the Park Hyatt Paris, Folks either love this place or leave scratching their heads wondering what the big deal is.  We fell into the former.
Damyel's (Paris): Delicious parisian parve chocolate and parisian macarons.
Ardely's (Paris):  Delicious parisian milk chocolate
Korcarz (Paris): Get a lemon meringue pie that you won't regret.
La Marina (Paris): Just for the fun that is the Farandole dessrt.
Gam Gam treats (Venice): Good pizza, great baked goods.
Cantina do Bero (Sao Paulo), go on a night where they have a "Rodizio" and you'll be treated to all you can eat pizza where any toppings you would like, but served by waiters and eaten with proper utensils.
Mehadrin (Sao Paulo)go on a Sunday when they have the all-you-can-eat grill of every kind of meat imaginable.
Matok (Sao Paulo): Pizza was just OK, but their soft serve and wrapped ice cream was delicious.
Goody (Sao Paulo): You're in for a treat if this dairy eatery has an all-you-can-eat while you're there.
Maichel (Buenos Aires): Lemon Mousse and Champagne with chocolate chunk Ice Cream in a homemade ice cream cone.  Divine.
Ba Li (Buenos Aires): Dulce de leche premium Ice Cream is heavenly.
Kokush (Buenos Aires): Chocolate covered dulce de leche tart.
Cafe Rimon Dairy (Jerusalem): I sometimes dream of the Hot Belgian Waffle dessert with white chocolate and Ben and Jerry's Ice Cream.
Cafe Rimon Meat (Jerusalem): Philly steak sandwich, though rumor is it may not be good anymore since they reopened?
Alter-Nativ (Tel Aviv): Excellent pasta dishes, chunky monkey milkshake, and a creme-brulee done better than anywhere.
Cafe C (Tiberias): The only worthwhile milchig place I found in the north.

OT but @dan you gotta try the deep fried garlic knots at shemtovs and while I must admit the bubba's burger w/pastrami at Sarah's is quite good I have a difficult time getting it into my mouth it's just so big

been to la marais- the wifey didnt like it so much
we love clubhouse but its not the same atmosphere of a nicer restuarant

maybe we will try solo
we were told that mikes bistro is very good but it seems that no one here agrees?

Highly rated and talked up restaurants that have left me at least mildly disappointed:

-Prime Grill (Manhattan)  After experiencing what this place could be in Beverly Hills this place is such a letdown.
-26 by Shiloh's (Los Angeles). Though dessert was awesome.
-Fish Grill Pico (Los Angeles). Dry fish, soggy pasta
-Shiloh's (Los Angeles). Far too inconsistent for the prices charged.
-Jeff's (Los Angeles). Don't get me wrong, this place IS GOOD.  But the legendary status that it has is undeserved.
-China Bistro (Fort Lauderdale)  Some of the worst chinese and appetizers I've ever tried though sushi was very good.
-Grill Time (North Miami Beach): It was good, but the hype was way too high for this place.
-Al Galope (Buenos Aires): Talked up to much to enjoy it.
-El Pasaje (Buenos Aires): C'mon, really now folks?

I think Mike's is excellent and deff deserves to be up there.
It sure was, easily would have been competition for my top 5.

Man! That place was really good!  Toronto can not seem to keep its restaurants open.  Miami Grill, King Solomon's, 398 west...and there are a few more that i can't remember.

The building clubhouse was in was sold maybe the new owner won't be able find a tenant and will invite then back probably not but let's hope
