Here's my NYC list (in no particular order):

Mike's Bistro - quickly becoming my favorite NYC restaurant. Ambiance is excellent, and the duck gnocchi is outstanding. The steak is also top 3 for me (they only have one to choose).
Le Carne - The first steak that blew my mind. Though as time went on, I became progressively more disappointed.
Deli Kasbah - Definitely not the best in the world, but their food consistently excellent, especially their steak strips and brazilian steak.
ETC - Yes, it's in Teaneck, not in NYC, but the last three times I've been there, it has rivaled Mike's and at least once has far surpassed it. The menu is constantly changing, but the best dish we had there was pomegranate steak strips. Un.real. I think I'm going there Sunday night for the "tasting menu," seven smaller dishes of the chef's best dishes. <licks lips>

Can't wait to try:

Tierra Sur - I hope to try in August.
Ladino Tapas - I hope to try in the coming weeks.

Random notes: The pepper steak at Le Marais, as I've mentioned before on DDF, is spectacular. Don't care for much else, or the ambiance for that matter. I found the sushi at TPG to be the best sushi I've had the pleasure of eating. I've been underwhelmed by Clubhouse, but can appreciate the ambiance... at times. Last, I heart Benny's pizza in Brooklyn. Man, that stuff be good.
Dont only take my word but Ladino Tapas was awful for me. Service was possibly the worst I ever had. Waited over an hour for appetizers, drinks didnt only came after multipe reminders (and they were just plain bad). Food was nothing to write home about. Location isint really central to anything.

+1 for Mike's Bistro -- perhaps the most unusual, "foodie" restaurant there is

+1 for Le Marais -- just consistently good steak.  Although unlike Dan (who is partial to the pepper encrusted entrecote), I only order the Hangar Steak (onglet).  We don't even look at the menu there any more....the Hangar Steak is just perfection IMO

Highly rated and talked up restaurants that have left me at least mildly disappointed:

-Prime Grill (Manhattan)  After experiencing what this place could be in Beverly Hills this place is such a letdown.
-26 by Shiloh's (Los Angeles). Though dessert was awesome.
-Fish Grill Pico (Los Angeles). Dry fish, soggy pasta
-Shiloh's (Los Angeles). Far too inconsistent for the prices charged.
-Jeff's (Los Angeles). Don't get me wrong, this place IS GOOD.  But the legendary status that it has is undeserved.
-China Bistro (Fort Lauderdale)  Some of the worst chinese and appetizers I've ever tried though sushi was very good.
-Grill Time (North Miami Beach): It was good, but the hype was way too high for this place.
-Al Galope (Buenos Aires): Talked up to much to enjoy it.
-El Pasaje (Buenos Aires): C'mon, really now folks?

Hands down no question about it I had worst restaurant experience of my life there. You couldn't pay me to eat there again.

ETA I have been to over one-hundred kosher establishments and that is the only one I refuse to go back to.

Would be cool if someone would gather everybody's reviews/ratings and come up with a top 10 list (#1 being the place most/more DDF'ers liking) for E"Y, [l'havdil] NYC, LA, Miami, etc..

and to have the reviews for each restaurant directly under their rating

and to constantly update as the reviews/ratings come in...

I alwasy left Shanghai feeling ill afterwards and told myself I would never go back...That is until someone treated us there ;)
I was actually pleasently surprised, Every dish that we ordered (all 4 of them) were actually delicious and I felt fine after! They were also very friendly (maybe ner management? Sure diff than before it seemed)

Shallots in Chicago has a Wagyu steak that will make you feel like you died and went to heaven. Ignore the unpronounceable sides that are supposed to come with it and ask for the homemade fries and onion rings with it instead. I don't eat sushi, but was told by others with me that it was the best sushi they ever had in their lives. The sweetbreads are also melt in your mouth good.

I've tried, really tried, to enjoy Pardes in Park Slope. It definitely is a foody experience, but I don't love it. I end up pounding their cream fries all meal.

Happy Family in Brooklyn has the best Chinese I ever had. Bit of a dump, but I've never gone wrong there.

Not sure exactly which hechsher, I do know there's a masgiach tmidi there. My stomach holds by whatever hechsher it has.
